    Pathogen Information

Streptococcus suis (S. suis) is a facultative anaerobic, Gram-positive bacterium that is commonly found in the respiratory and digestive tracts of pigs. It is an important pathogen that causes severe and often fatal infections in swine worldwide. S. suis was first isolated in Denmark in 1968 from pigs with meningitis.

    S. suis can be grouped into 35 serotypes based on the composition of its capsular polysaccharides (CPSs). CPSs are considered the most important virulence factor of S. suis, as they protect the pathogen from host immune defenses, promote bacterial adhesion to host tissues, and inhibit phagocytosis. Among the 35 serotypes, serotype 2 (SS2) is the most prevalent and virulent strain affecting both pigs and humans.

The main route of transmission of S. suis is through direct contact with infected pigs or pork products. S. suis can also be transmitted through the aerosol route, although this is less common. The incidence of S. suis infection varies widely across different regions and depends on various factors, such as farming practices, animal health management, and food processing.

In pigs, S. suis can cause a wide range of diseases, including meningitis, septicemia, arthritis, pneumonia, endocarditis, and reproductive disorders. S. suis infection in pigs usually occurs in outbreaks and can lead to significant economic losses in the swine industry due to mortality, reduced growth rates, and treatment costs.

    S. suis is also a zoonotic pathogen that can cause severe infections in humans who come into direct contact with infected pigs or pork products. Human infections with S. suis are usually sporadic but can occur in outbreaks, particularly in countries with a high prevalence of S. suis in pigs. In humans, S. suis infection can cause systemic manifestations, including meningitis, septicemia, endocarditis, arthritis, and pneumonia. The mortality rate of S. suis infection in humans is around 10%.

Diagnosis of S. suis infection can be challenging, as clinical manifestations can be variable and nonspecific. Several laboratory methods are available for the detection of S. suis infection, including culture-based methods, serological tests, and nucleic acid amplification tests (NAATs). Culture-based methods involve isolating the bacteria from clinical specimens such as blood or cerebrospinal fluid, which is the gold standard for diagnosis. Serological tests detect S. suis-specific antibodies in the blood or cerebrospinal fluid, which can provide evidence of recent or past infection. NAATs target specific genes such as the 16S rRNA gene, the cps gene cluster, and the suilysin gene for the detection of S. suis DNA in clinical samples.

In addition to diagnosis, prevention and control of S. suis infection in both pigs and humans are essential. Several strategies are available to prevent S. suis infection in pigs, including good farming practices, vaccination, biosecurity measures, and antibiotic therapy. In humans, the prevention of S. suis infection involves the avoidance of high-risk activities, such as pig farming and pork processing. Proper food handling and cooking are also important to prevent S. suis contamination.

In conclusion, S. suis is a significant pathogen that affects both pigs and humans worldwide. The pathogenicity of S. suis is attributed primarily to its capsular polysaccharides, which protect the bacterium from host immune defenses. S. suis infection can cause various diseases in both pigs and humans, and diagnosis of S. suis infection can be challenging. Prevention and control of S. suis infection require coordinated efforts among various stakeholders, including farmers, veterinarians, public health officials, and policymakers.
